Living Will

It states that if you were to become incapacitated and show no signs of recovery. unconscious situation and show no signs of improvement, you would like all life-prolonging measures to stop. This decision would be in compliance with the medical facility’s Board of Ethics Committee, formed at every hospital. This document is completed so that this difficult decision is not placed on children or loved ones.

Special Needs Trust

A Special NeedsTrust can be set up for someone who needs to have their assets protected , so as not to interfere with their Medicaid benefits.

Revocable Living Trust

Medical Directives

These documents are generally done along with a Health Care Power of Attorney. While that document gives an individual the authority to make medical decisions for you, this document limits that authority to specific matters.

Medical Authorization for Minors

Deed Preparation
